ASSISTIVE TECHNOLOGY & HOME MODIFICATIONS (AT-HM) SCHEME
The AT-HM scheme provides funding for home modifications and assistive technology to help older Australians live safely and independently at home.
WHAT IS THE AT-HM SCHEME?
The Assistive Technology and Home Modifications (AT-HM) scheme is part of the Australian Government's Support at Home Program. It provides funding to help older Australians:
- Remain living safely and independently in their own homes
- Reduce the risk of falls and injuries
- Access all areas of their home with ease
- Maintain their quality of life and independence
The scheme covers both assistive technology (equipment) and home modifications (structural changes) that are reasonable and necessary to support your independence.
SEPARATE FUNDING
AT-HM funding is separate from your regular Support at Home budget. This means:
- You don't need to use your regular support budget for modifications
- AT-HM funding is specifically allocated for equipment and home changes
- You can access both types of funding simultaneously
- Your regular support services continue unaffected
This separate funding ensures you can make necessary home modifications without impacting your other support services.
ASSESSMENT IS KEY
Before any modifications can be approved, you'll need an assessment from a qualified professional, typically an Occupational Therapist. The assessment will:

The Occupational Therapist will assess your mobility, daily activities, and home environment to identify what modifications would be most beneficial.
RECOMMEND SOLUTIONS
Based on the assessment, the Occupational Therapist will recommend specific modifications that are reasonable and necessary for your situation.
SUPPORT FUNDING REQUESTS
The Occupational Therapist's report provides the clinical justification needed for your funding application to be approved.

FUNDING TIERS
The AT-HM scheme has different funding tiers based on the complexity and cost of the modifications:
TIER 1
LOW COST

MODERATE COST
More complex modifications like ramps, bathroom alterations, and widened doorways.
TIER 3
HIGH COST
